School Social Work Services

School social work services are available to all students. Services are not intended to take the place of an outside counselor or therapist and should not be used as such. School social work services and subsequent follow up(s) are intended for social emotional learning, crisis situations, disciplinary referrals as specified in a disciplinary plan, preventative measures and other issues that may arise that affect a student’s ability to learn and function in the school environment. School social work services may be delivered on an individual, group or classroom basis. If ongoing and regular school social work services are recommended or requested by school personnel, a parent or a student; a parent needs to provide specific, signed, informed consent unless the child is 18 years old or older. In cases of discipline referrals and crisis, signed consent is not necessary. All school social work services are confidential and cannot be disclosed or shared unless the student is being harmed, plans on or is harming someone else, or is harming themselves.

You can also print off the form, complete it, and return it to the Elementary or JH/HS office.
